CUKR card for PESEL UKR holders

A 3-year CUKR card: your MOS application, completed with an expert

We check your UKR dates, prepare the files and complete the application with you in one online meeting. Applications are accepted until 4 March 2027.

Who can get the CUKR card

There are three conditions, and all of them can be checked by dates before you apply. If one isn't met, we tell you straight away and suggest other grounds.

    UKR status on 4 June 2025

    On that day you already held valid UKR status (PESEL UKR).

  2. 02

    365 days without a break

    Your UKR status lasted continuously for at least 365 days. Leaving Poland for more than 30 days interrupts the status.

  3. 03

    Status valid on the application date

    Your UKR status is active on the application date, and your PESEL data is complete: identity confirmed, fingerprints given.

Applications are accepted from 4 May 2026 to 4 March 2027, electronically via MOS only. After that date the CUKR card is no longer available and only the general grounds remain.

How the MOS application works

Five steps. At each one you can see what you do and what we do.

    Book the meeting

    You: choose a time on cal.com and pay for the meeting.We: send you a questionnaire and a list of what you'll need at the meeting.

    Dates and data check

    You: fill in the questionnaire and prepare a digital photo.We: check the three conditions and your PESEL data before the meeting. If you don't qualify, we tell you in advance.

  3. 03

    Google Meet session

    You: fill in and sign the application in your own MOS account.We: go through every field with you and check the data before it is sent.

  4. 04

    State fee and case registration

    You: pay the state fee of 340 zł + 100 zł directly to the state.We: give you a checklist: what to keep and what to do after the case is registered.

  5. 05

    Decision and card

    You: receive the decision and collect the card within 60 days.We: with case support, follow the case, receive the office's letters and remind you of deadlines.

Without case support, you handle the case yourself after the meeting, following the expert's checklist.

What changes when you move from UKR

The card gives you more stability, but some UKR benefits end. It's better to know this before you apply.

Safety

  • We never ask for your Profil Zaufany login or password.
  • You pay the state fee yourself, from your own account.
