Key requirements
- Polish nationality of ancestors: at least one parent or grandparent, or two great-grandparents, were of Polish nationality (narodowość polska)
- Birth certificates, marriage certificates, and other vital records confirming the family connection to the Polish ancestor

Which documents confirm Polish roots
To confirm Polish ancestry, documents that directly state the ancestor's Polish nationality are used. Ancestry can be confirmed through one parent, one grandparent, or two great-grandparents of Polish nationality.
